We take confidentiality very seriously.

All evaluation and treatment information is confidential and can only be released to specified individuals or agencies with your written release. Massachusetts & Rhode Island law dictates some important exceptions to confidentiality. A patient presenting with imminent danger to self/others is one such exception. Confidentiality with adolescents (fourteen and older) is a sensitive issue that must take into consideration the child’s developmental need for privacy, with the parents’ right to be informed of their child’s course of treatment. We are committed to finding the best balance.

When you arrive at the office, you will be asked to sign the Notice of Privacy Practices (HIPPA) form. Additional information about HIPPA will be made available upon request.

Patient/Family Concerns

We aim to provide you with the highest quality care in a pleasant and professional atmosphere. If you have concerns about any aspect of our services, please feel free to inform Onyebuchi Ogbo, PMHNP.

Payment and Billing

Payment in full is due for each appointment at the time of treatment. For your convenience, we do accept MasterCard, Visa, American Express, Discover, Cash, Debit, and Check. Checks should be made payable to “Good Path Health Services.”

We require 48-hour notice of cancellation for ALL appointments. In the case of a bonafide emergency, exceptions will generally be made. When you do not keep an appointment and do not contact us with 48 hour notice, it is a lost appointment time that could have been offered to another patient.

Insurance

Medicare: Good Path Health Services accepts patients with Traditional Medicare Primary. We DO NOT accept Medicare Advantage plans. If you are unsure of which plan you have, our friendly staff will gladly assist you in determining your Medicare Plan coverage.

Out-of-Network: If you participate in a major medical insurance plan and wish to utilize your benefits, we can assist you as an out-of-network provider. Because plan coverage, deductibles, and co-payments differ, we encourage our patients to consult with their insurance companies prior to their visit to have a clear understanding as to what services may or may not be covered.

If your insurance is not listed here or you are unsure, please feel free to call us at (339) 502-0871 or fill out an intake form.

Please have your insurance card available when you call.
